Immunization Compliance Overview
Per 新澳门六合彩内幕信息Policy 33-002, it is mandatory for 新澳门六合彩内幕信息students to submit all required immunization documentation and/or the completed 新澳门六合彩内幕信息Medical History Form prior to course registration. Tip: Your military records may provide vaccination dates, or you may submit a lab report for blood tests performed within the last 5 years to document your immunity.
Methods to upload records
You may submit/upload your documents via Web Submission or MyBullsPath. Documents may be pdf, jpg, or png.
